Job summary

Live Well Homecare Agency, LLC seeks a Registered Nurse to provide high-quality patient care within a collaborative healthcare team. You will assess, plan, implement, and evaluate care to promote optimal outcomes while acting as a patient advocate and educator.

You will collaborate with care team members to coordinate plans, participate in performance improvement, maintain professional competence, mentor colleagues, and uphold confidentiality and safety policies.

Qualifications

• Associates, Bachelors, or Masters in Nursing preferred.
• Current RN licensure (single or multi-state compact) and BLS certification required.
• Strong communication, clinical judgment, and teamwork skills.

Responsibilities

• Assessing, planning, implementing, and evaluating patient care to promote optimal outcomes.
• Serving as a patient advocate by addressing concerns, educating patients and families, and ensuring satisfaction.
• Collaborating with care team members to coordinate care plans and communicate effectively.
