Job summary

ChristianaCare’s Primary Care at Linden Hill in Wilmington, Delaware is seeking a Registered Nurse II to deliver high-quality patient care across age groups in both in-person and virtual settings.

The role includes wellness visits, care management, point-of-care testing, and triage, with a rotating schedule across regional practices. Opportunities for remote work after orientation are available.

Qualifications

• Must be a graduate of an approved nursing program.
• Current RN licensure in DE/MD/PA/NJ depending on location.
• Minimum 2 years RN experience; BSN preferred.
• BLS certification required.

Responsibilities

• Provide age-appropriate, patient-centered care from pediatric to geriatric populations, in virtual and in-person settings.
• Deliver wellness visits, care management, point-of-care testing, and triage.
• Serve as liaison among patients, providers, and staff; coordinate workflow and patient flow.
• Mentor staff, participate in quality and safety initiatives, and support process improvements.
