Job Description

Functions independently and is accountable for utilizing the nursing process. Coordinates patient care with the Multidisciplinary Team, delegates tasks appropriately, and identifies hospital resources. Serves as a clinical resource for new employees and demonstrates positive leadership within the unit or department.

In Virtual Setting

Functions independently, interprets data with increasing judgment, and prioritizes appropriately. Initiates actions to improve nursing practice and patient care. Acts as a resource to colleagues, delegates tasks, and assumes preceptor responsibilities. Utilizes hospital resources and provides tele-nursing care to a diverse patient population across departments within SLHS. Key features include patient advocacy, prevention, telecommunication, proactive monitoring, evidence-based care, clinical decision support, and compassionate, ethical care. Monitors assigned patients via intelligent software, alerts staff to changes, and partners with bedside teams to address concerns, aiming to identify early signs of issues and reduce complications.

Responsibilities
  • Monitor intelligent software and vital signs for changes and trends.
  • Communicate effectively in tense situations.
  • Leverage experience to interpret trending data for this population.
  • Partner with bedside staff for monitoring.
  • Provide early detection of potential problems.
  • Notify bedside team and assist in activating resources.
  • Serve as an additional monitoring resource with intelligent software.
  • Identify early signs to reduce complications.
Community Hospital Setting
  • Manage care within established nursing standards.
  • Prioritize needs with a caring attitude.
  • Stabilize patients and manage life support in collaboration with physicians.
  • Gather detailed patient histories and symptoms.
  • Perform lab testing and report adverse treatments.
  • Prepare patient meals.
  • Participate in QAPI activities and maintain standards.
Hospital In Your Home Setting
  • Provide in-home nursing services.
  • Assess physical and psychosocial needs.
  • Work with the healthcare team to optimize outcomes.
  • Execute orders, notify physicians of changes.
  • Maintain clinical competencies and participate in training.
  • Educate and counsel patients and families.
  • Document visits and interventions.
  • Follow legal and institutional policies.
  • Perform other duties as assigned.
Ambulatory Surgery Center (ASC) Setting
  • Provide professional nursing care following standards.
  • Handle telephone triage and patient flow.
  • Schedule urgent appointments and provide patient education.
  • Delegate tasks appropriately.
  • Serve as a subject matter expert in patient care and testing.
  • Work independently or in teams.
  • Deliver excellent customer service.
  • Perform additional office duties as needed.
  • Collect patient history, vital signs, and lab samples.
  • Report adverse treatments or medications.
  • Participate in accreditation and compliance activities.
Job Requirements
  • 2 years of applicable nursing experience.
  • Basic Life Support (AHA or Red Cross), Neonatal Resuscitation Program (NRP), RN license.
  • Associate Degree in Nursing.
