Sutter Health is seeking an RN for a full-time position at the CP​MC-California Pacific Med Center in San Francisco, CA. The role includes direct patient care, patient/family education, and coordination of transitions of care in an acute setting.

Hybrid collaboration with physicians and care teams ensures a holistic approach to patient outcomes. Required are an accredited RN program completion or RN license, plus BLS.

Qualifications

• Graduate of an accredited school of nursing OR RN license evidences completion of a RN program
• RN-Registered Nurse license for the state where care is provided
• BLS-Basic Life Support Healthcare Provider
• 2 years of recent relevant experience.

Responsibilities

• Assess, coordinate, deliver and evaluate nursing care in an acute patient setting.
• Provide direct patient care and patient/family education.
• Coordinate transitions of care and maintain collaboration with patients, families, physicians, and healthcare team.
• Adhere to hospital policies, industry standards, and regulations to promote optimal patient experience.
