The Registered Nurse BSN embodies the attributes of the Relationship Based Care model and performs to PPM core functions. Under the direction of the department leadership, the nurse reflects the mission, vision, and values of NM, adheres to the State of Illinois Nurse Practice Act, the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and regulatory standards.
Responsibilities
Patient and Family Care
- Performs comprehensive assessments and documents findings.
- Communicates patient condition changes promptly.
- Uses nursing diagnosis and assessment data to set measurable goals and modify care plans accordingly.
- Prioritizes patient needs and collaborates with patients, families, and healthcare team for care planning.
- Evaluates patient responses to interventions and adjusts plans as needed.
- Assesses discharge needs and makes referrals, including for palliative care.
- Completes discharge instructions, medication reconciliation, and educates patients/families on resources.
Patient Education, Ethics, and Diversity
- Supports patient/family/spiritual/cultural needs in education and care planning.
- Uses teach-back methodology and distributes educational materials.
- Respects cultural differences and adheres to ethical standards.
Evidence-Based Practice and Research
- Implements interventions based on evidence and best practices.
- Engages in professional development and quality improvement initiatives.
Safe Environment
- Administers medications safely using barcode technology.
- Ensures a safe environment and effective communication during hand-offs.
- Participates in quality metrics and safety initiatives.
Colleague Relationships
- Leads and directs care team members effectively.
- Serves as a resource and mentor for staff and students.
- Participates in peer evaluations and team collaboration.
Resource Utilization and Technology
- Documents appropriate levels of care and adheres to resource policies.
- Utilizes and advocates for new technology and system tools.
Self-Development
- Participates in professional development activities and obtains relevant certifications.
Qualifications
Required:
- Bachelor of Science in Nursing (BSN)
- Current Illinois RN license
- Basic Life Support certification
Preferred:
- Specialty certification
- Strong interpersonal and collaborative skills
- Analytical abilities
- ADN with commitment to obtain BSN within 3 years
