Position Summary

Responsible for planning, organizing, directing, implementing, and evaluating all aspects of resident care to meet residents' needs and maintain care in accordance with State and Federal laws and community policies.

  • Communicate with nursing staff, physicians, pharmacists, family members, etc., regarding residents’ status.
  • Listen to concerns from residents, families, or physicians; initiate corrective actions or report concerns as needed.
  • Attend Resident Care conferences; maintain accurate and relevant Resident Care Plans.
  • Prepare, administer, and document medications per physician orders; order medications as necessary and replace expired medications.
  • Document patient care issues appropriately and report them as required.
  • Recognize signs and symptoms; use judgment to determine appropriate treatment or procedures.
  • Supervise and direct ancillary personnel; initiate corrective actions when needed.
  • Perform other duties as assigned.
Basic Qualifications & Experience
  • Advanced training in geriatrics preferred.
  • 1-year experience in a long-term care facility preferred.
  • 1-year supervisory experience preferred.
  • Proficient in Microsoft Office applications including Excel, Word, and Outlook.
  • Excellent customer service skills.
  • Ability to read, write, and speak English fluently.
Working Conditions / Physical Requirements
  • Moderate physical activity; handling objects up to 50 pounds occasionally and 20 pounds frequently.
  • Possible exposure to chemicals, bloodborne pathogens, and infectious diseases.
  • Sit, stand, bend, and move intermittently during work hours.
  • Frequent interruptions and interactions with residents, families, visitors.
