Job summary

Rochester Regional Health is seeking an experienced Registered Nurse to join the Transitional Care team at Park Ridge Living Center. This evening or night shift position offers 12-hour shifts, 40 hours per week, and opportunities for leadership roles within a CMS 5-Star facility.

Responsibilities include patient care, care plan development, EHR management and regulatory compliance. RN license in NY and BLS are required; BSN preferred.

Qualifications

• Diploma or Associate Degree in nursing is required.
• Registered Nurse license in New York State and BLS certification are required.
• AS Nursing is listed as required/education.
• Bachelor's degree is preferred.

Responsibilities

• Provide comprehensive nursing care by executing the nursing process in collaboration with physicians and the health care team.
• Develop and document individualized care plans with interdisciplinary support; communicate patient status and plans clearly.
• Manage electronic health records with accurate documentation and timely updates.
• Ensure compliance with department, system protocols, and regulatory requirements.

RESPONSIBILITIES:

• Patient Care & Service. Promote and restore patients' health by completing the nursing process; collaborating with physicians and multidisciplinary team members; performing various treatment procedures; providing physical, educational and emotional support to patients, friends and families; supervising assigned team members
• Planning & Communication. Develop and document individualized care plans customized for each patient's unique needs, with support from the interdisciplinary health team as needed; maintain effective communication to convey patient health status, treatment plans and progress
• Electronic Health Record (EHR) Management. Demonstrate proficient use of an EHR - including accurate patient and provider documentation and communication
• Compliance. Adhere to required department and system protocols, regulations (local, state, federal) and education requirements

REQUIRED QUALIFICATIONS:

• Diploma or Associates Degree in nursing required
• Registered Nurse license in New York State. Basic Life Support (BLS) certification

PREFERRED QUALIFICATIONS:

• Bachelors Degree.

PHYSICAL REQUIREMENTS: Moderate - Clinical - Moderate clinical roles are patient facing roles that require continual movement, including standing, walking, and lifting 20-50 lbs., along with pushing and pulling medical equipment and assisting with patient positioning. Employees perform frequent bending, twisting, and reaching while completing fine motor tasks in busy or confined clinical environments. High sensory acuity is essential for monitoring patient status and responding to alarms or safety concerns. Emotional and cognitive demands are substantial, involving decision-making under pressure, responding to emergencies, and maintaining sustained focus. Environmental exposures include infectious agents, bodily fluids, chemicals, and radiation. TB testing, mask-fit testing, and physical exams are common requirements.

PAY RANGE: $38.66 - $51.91
