Hackensack Meridian Health in Brick, NJ is seeking a Registered Nurse for the Hospital at Home program. This per diem, day-shift role focuses on delivering evidence-based nursing care to a diverse patient population.

You will document assessments and plans electronically, administer medications with bar-scanning and maintain patient and family education. A NJ nursing license and BLS/CPR are required, with BSN pursued within three years.

Qualifications

• Associate in Nursing or diploma required; BSN in progress or willing to pursue within three years of hire.
• Six months of relevant nursing experience or active Nurse Residency participation.
• Current New Jersey Nursing license and BLS/CPR certification within 60 days of hire.

About This Role

The Registered Nurse functions as a caregiver for a specific patient population to achieve desired outcomes. This position focuses on expanding knowledge and skills while providing effective direct care as part of an interdisciplinary team to a variety of complex patients.

What You’ll Do

• Deliver nursing practices according to evidence-based research and standards of care.
• Administer medications utilizing advancements in bar-scanning and documentation technologies.
• Document patient assessments, plans of care, interventions, and evaluations electronically.
• Provide patient and family-centered education aimed at optimizing health and wellness.
• Facilitate multi-disciplinary discharges to ensure safe transitioning and continuum of care.
• Provide care within a patient’s home as part of the Hospital at Home program.
• Facilitate the intake/admission process when operating in the hospital.

Requirements

Education

• Associate in Nursing or diploma of Nursing degree required; BSN in progress or willingness to acquire within three (3) years of hire.

Experience

• Six (6) months of relevant nursing experience required or active participation in Hackensack Meridian Health’s Nurse Residency Program.
• Three (3) years of med/surg or Telemetry nursing experience preferred.

Licenses & Certifications

• Current and valid New Jersey Nursing license required.
• BLS/CPR certification by American Heart Association required within 60 days of hire.
