Responsibilities

• Deliver nursing care based on the latest evidence, policies, and standards to ensure safe, individualized patient care.
• Communicate continuously with patients, colleagues, and the healthcare team regarding patient status, care, and treatment plans.
• Administer medications using advanced bar-scanning and documentation technologies to ensure safety.
• Document assessments, care plans, interventions, and evaluations electronically at the point of care.
• Provide patient and family education to promote health, wellness, and disease management.
• Coordinate multidisciplinary discharges to ensure safe transition and ongoing care.
• Perform home visits as part of the Hospital at Home program for acute care, and facilitate patient intake and admission processes.

Qualifications

• Current and valid New Jersey RN License.
• BLS/CPR certification from the American Heart Association within 60 days of hire.
• BSN required.
• Preferred: 2 years of acute care experience.
• Comfortable conducting home visits.
• Reliable transportation.
