We seek a Registered Nurse for our Hospice Home Care McDowell Team to ensure high-quality, efficient patient care. If you value patient care as much as we do, we want you to apply!

Job Summary and Qualifications

The Case Manager (CM) ensures high-quality, patient-centered care by managing rehabilitative care to achieve optimal outcomes. The CM provides, coordinates, and directs care tailored to each patient's needs, collaborating with the Rehab Program Director and Facility Case Management Director.

Responsibilities include:

  • Providing program orientation to patients, families, and caregivers, including role, philosophy, stay and discharge criteria, Medicare and insurance benefits, grievance procedures, treatment plans, and patient rights.
  • Coordinating patient/family/caregiver education and promoting participation in planning and goal setting through Family Conferences and other interactions.
  • Implementing the patient's treatment plan, supporting their strengths, needs, and preferences, and involving them throughout the rehabilitation process.
  • Documenting discharge planning evaluations and psychosocial assessments, communicating social, financial, and discharge needs.
  • Promoting positive patient interactions that enhance service quality.

Qualifications:

  • Basic Cardiac Life Support certification within 30 days of employment.
  • Driver's License within 2 years of employment.
  • Registered Nurse licensure within 2 years of employment.
  • Associate Degree.
