What You Will Do

• Practice independently in the community as part of an interdisciplinary care team.
• Deliver personalized nursing and care management to patients in their homes or care facilities.
• Evaluate evolving patient needs and respond with care plan adjustments.

Qualifications

• Current license to practice as a Registered Nurse in New York State
• Minimum of one year nursing experience in a medical/surgical environment
• Valid driver's license or NYS Non-Driver photo ID card may be required
• Wound Care Certification required or willingness to obtain within one year of hire
• Certification in Hospice and Palliative Care Nursing required
• Bilingual skills in Cantonese or Mandarin required

Pay Range

USD $112,209.00 - USD $138,409.00 /Yr.
