Registered Nurse - Hospice Acute Care Coordinator

GENERAL SUMMARY:

Functions as a care coordinator for Hospice within the hospice setting. Assesses and identifies patient needs in accordance with the hospice concept of care. Coordinates with interdisciplinary team to provide complete patient/family care and referral. Provides pain management and symptom control. Maintains the continuity of care by ensuring patient needs are met.

EDUCATION/EXPERIENCE REQUIRED:

• Requires an Associate's Degree or Diploma in Nursing.
• Understanding and acceptance of the Hospice concept of care, with a working knowledge of current pain and symptom management.
• Interpersonal skills to positively represent hospice to referrals.
• Flexibility to adapt policies and procedures to meet hospice patient and family needs.
• Previous hospice experience preferred.

CERTIFICATIONS/LICENSURES REQUIRED:

• Current licensure in the State of Michigan as a Registered Nurse.
• Hospice certification preferred.
