Henry Ford Health is seeking a care coordinator for Hospice to assess patient needs, coordinate with the interdisciplinary team, and ensure comprehensive care with effective pain management.
The role requires an RN with hospice experience and strong interpersonal skills, capable of working independently within our hospital-based hospice program. Day shift in Detroit facility.
Functions as a care coordinator for Hospice within the hospice setting. Assesses and identifies patient needs in accordance with the hospice concept of care. Coordinates with interdisciplinary team to provide complete patient/family care and referral. Provides pain management and symptom control. Maintains the continuity of care by ensuring patient needs are met.

Qualified candidate will have RN degree, one year of Med/Surg experience, Hospice experience is preferred. Candidate should be able to work independently but also cohesively with team members. Qualified candidate should also be able to build strong relationships with key stakeholders including but not limited to, hospital Case Managers, providers, and unit staff. Our department will work as Hospice Liaisons within the hospital setting, including but not limited to, evaluating and assessing patients for hospice inpatient and also assisting with complex hospice consults/referrals.
