Job summary

Munson Healthcare is seeking a registered nurse in Michigan to manage patient care from admission through discharge using a framework of assessment, planning, coordination, and evaluation.

The role includes comprehensive assessments, developing individualized plans, and delivering skilled nursing services across preventative, rehabilitative, palliative, and end-of-life needs, while coordinating care with an interdisciplinary team. Requires Michigan RN license and travel to patient homes.

Qualifications

• Licensed RN in Michigan
• Experience in acute care, home care, or hospice preferred
• Proven health assessment, organizational, and interpersonal skills with the ability to make independent judgments directing patient care.
• Able to meet Home Health and Medicare standards through criminal background check and fingerprinting.

Responsibilities

• Manage an assigned caseload from admission through discharge.
• Complete comprehensive assessments (including OASIS when required), develop and update plans of care, and provide skilled nursing services across preventative, rehabilitative, palliative, and end-of-life needs.
• Educate patients and families on disease processes, symptom management, and self-care, and coordinate care with interdisciplinary team members.
• Supervise LPNs and paraprofessionals, communicate changes in patient condition to physicians and team members, secure equipment and supplies, and maintain timely documentation in the EMR.

A Day in the Life

Manages an assigned caseload from admission through discharge using a framework of assessment, planning, coordination, and evaluation.

Responsibilities include completing comprehensive assessments (including OASIS when required), developing and updating individualized plans of care, and providing skilled nursing services across preventative, rehabilitative, palliative, and end-of-life needs.

Educates and counsels patients and families on disease processes, symptom management, and self-care, and coordinates care with interdisciplinary team members such as Home Health Aides, Therapies, Social Work, Chaplaincy, and Bereavement Services.

This position also supervises LPNs and paraprofessionals, communicates changes in patient condition to physicians and team members, secures necessary equipment and supplies, and maintains thorough, timely documentation within the electronic medical record.

Qualifications

What's Required

Licensed RN in the State of Michigan

Experience in acute care, home care, or hospice preferred

Proven health assessment, organizational, and interpersonal skills with the ability to make independent judgments directing patient care.

Able to meet Home Health and Medicare standards through criminal background check and fingerprinting.

Possess current Michigan motor vehicle license, ability to drive a car and an insured vehicle capable of transport to patient homes or other offices in various weather conditions.

Additional Information
