Job summary

Heritage Home Health & Hospice is seeking a Registered Nurse to deliver compassionate home-based care in the Pocatello, ID area. You will visit patients at home, coordinate with families and the health care team, and ensure adherence to agency, state, and federal policies.

The RN will consult with primary care physicians, verify medication profiles, and participate in required meetings while maintaining high-quality documentation and patient-centered care.

Qualifications

• Be currently licensed as a Registered Nurse in Idaho.
• A minimum of one year of clinical experience is preferred but not required.
• Demonstrate knowledge and skill in nursing practice.
• CPR professional certification.
• General knowledge of computers and the ability to learn industry-specific software and documentation applications.
• Possess a valid state driver’s license and automobile liability insurance.

Responsibilities

• Provide home health visits and coordinate care with the patient and family.
• Serve as the primary contact for all care disciplines and ensure accurate medication profiles.
• Consult with the patient’s physician, participate in required meetings, and follow agency policies and procedures.

Responsibilities

Heritage Hospice Registered Nurses (RNs) work with patients, their families, and other healthcare professionals in the patient’s place of residence. In addition to performing visits and completing coordination of care, the RN is the point of contact for all disciplines involved with providing care to the patients. The RN follows agency, state, and federal policies and procedures. The RN will consult with the patient’s primary care physician, ensure medication profiles are accurate, and attend meetings as required.

Registered Nurses (RNs) must meet the following requirements:
