Responsibilities

The Registered Nurse Case Manager assumes full nursing responsibility for delivering the Plan of Care for each hospice patient. Continuously evaluates personal and professional performance and makes necessary adjustments to improve productivity and quality of care. Adheres to hospice policies, procedures, goals, and objectives to ensure quality patient care.

• Manage the delivery of the Plan of Care for each hospice patient.
• Evaluate and improve personal and professional performance regularly.
• Support and adhere to hospice policies and procedures to ensure quality care.
• Make independent, critical decisions regarding patient care in a cost-effective manner without compromising quality.
• Develop and implement patient care plans collaboratively, based on scientific nursing principles, promoting physical, psychosocial, emotional, and spiritual well-being.

Education and Experience

Experience Requirements

• Minimum one year of clinical experience.

License Requirements

• Current RN licensure in the practicing state.
• Valid CPR certification.
• Valid driver’s license, vehicle insurance, access to reliable transportation, or public transit.
