The Lieutenant Colonel (LTC) Charles S. Kettles VA Medical Center is seeking a Full-Time Registered Nurse (RN) - Home Telehealth (HT) Care Coordinator for Ambulatory Care.
Qualifications
Basic Requirements:
- English Language Proficiency
- In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless proficient in basic written and spoken English.
- Graduate of an accredited school of professional nursing (ACEN or CCNE) at the time of program completion.
- Or individuals attending a master's level bridge program in nursing with coursework equivalent to a bachelor's degree, eligible for temporary appointment and later conversion upon graduation.
- Foreign nursing graduates with current, unrestricted registration; degree equivalency validation required for higher education.
- Current, full, active, unrestricted registration as a professional nurse in the U.S. or its territories.
- Graduate Nurse Technician (GNT) exception: candidates without licensure may be appointed temporarily while pursuing licensure.
- Grade determinations detail the scope, education, and experience required for each nursing level, from Nurse I to Nurse III, including specific educational and experience criteria.
- Physical requirements include visual acuity, hearing, speech, manual dexterity, and ability to perform physical activities such as walking, standing, and transferring patients.
- Exposure to infectious materials and potentially combative patients may occur; protective clothing and safety measures are required.
Duties
- The RN Home Telehealth Care Coordinator manages a panel of patients, ensuring timely, appropriate, high-quality, and cost-effective care.
- Collaborates with healthcare team members, clinics, community agencies, and leadership.
- Provides assessment, coordination, planning, and ongoing evaluation of healthcare services for veterans with various health conditions.
- Reports to the Connected Care Program Manager and coordinates care across the continuum for conditions like hypertension, CHF, COPD, diabetes, PTSD, dementia, depression, bipolar disorder, schizophrenia, and weight management.
