Job summary

BAYADA Home Health Care is seeking a Registered Nurse for home health across Northumberland County. Flexible schedules and 1:1 care with a strong work/life balance.

Per Diem, Part-Time, or Full-time opportunities available. Sign-on bonus up to $15,000 for full-time roles. Ideal candidates hold an active RN license and have 1+ year of clinical experience.

Qualifications

• A current RN license in STATE.
• Minimum of one year of clinical (medical/surgical) experience.
• Home care experience preferred but not required.
• Graduation from an accredited nursing program.
• Ability to travel to cases as assigned.

Responsibilities

• Follow a designated care plan per patient needs.
• Make home visits to clients within designated territories.
• Administer medications, perform wound care, treatments and procedures.
• Monitor client conditions and report changes to supervisor.
• Document and follow doctors' orders and care plans.
• Perform client assessments and coordinate case management.
• Record observations and interventions in the EMR.

Registered Nurse (RN) Responsibilities:

• Follow a designated care plan in accordance with patient's needs
• Make home visits to clients in designated geographic territories
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures
• Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
• Follow up with, execute, and properly document doctors' orders
• Perform client assessments as necessary
• Case management and coordination
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet

Qualifications for a Registered Nurse (RN):

• A current license as a Registered Nurse in STATE.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care experience strongly preferred, but not required.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.
