Sunbelt Staffing seeks a dedicated Registered Nurse for a 13-week, full-time contract delivering skilled nursing care in patients' homes within a field-based home health setting.

You will perform OASIS assessments, develop care plans, and administer wound care, IV therapy, and medications, while documenting thoroughly in HCHB and collaborating with the care team. Travel daily within territory with mileage reimbursement.

Qualifications

• Active RN license in good standing in the state of practice.
• At least 1 year of recent home health nursing experience (field experience preferred).
• Proficiency with Home Care Home Base (HCHB) and OASIS assessments.
• Reliable transportation with valid driver's license and auto insurance.

Responsibilities

• Perform in-depth OASIS assessments and develop tailored care plans.
• Deliver skilled nursing interventions such as wound care, IV therapy, chronic disease management, and medication administration.
• Monitor patient progress vigilantly, communicate status changes, and collaborate closely with the care team and physicians.
• Educate patients and families about disease management, medication regimens, and preventive strategies.
• Ensure meticulous, timely documentation using Home Care Home Base (HCHB).
• Collaborate with multidisciplinary team members, including therapists, social workers, and aides.
• Attend case conferences, staff meetings, and participate in ongoing education.
• Uphold rigorous standards for documentation and care delivery in compliance with regulations.

Embark on a rewarding 13-week full-time contract providing skilled nursing care in a field-based home health setting. In this impactful role, youll deliver compassionate, comprehensive care to patients in their own homes, helping them achieve optimal outcomes and quality of life.

Schedule & Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Daily travel to patient homes within the assigned territory (mileage reimbursement provided)
• Caseload: 30 points per week (typically 35 patient visits per day)

$1,742 - $1,843/WeekEstimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
