Responsibilities

• Implement treatment and rehabilitative strategies as outlined in the patient care plan, including medication management, rehabilitation treatments, coordination of medical needs, and budget monitoring.
• Maintain thorough and timely documentation of care provided for accurate patient records and billing.
• Conduct assessments and approve plans of care and service delivery methods.
• Follow consumer health plans, promote consumer choice, and facilitate consumer independence.
• Perform competency and skills evaluations on employees providing direct care.
• Other tasks as assigned.

Qualifications

• Must be at least 21 years old.
• Active Registered Nurse (RN) license in good standing in the state of operation.
• Bachelor’s Degree in nursing or related health and human services field from an accredited college or university OR an Associate Degree in nursing from an accredited two-year program.
