Amberwell Health in Kansas is seeking a Health at Home Registered Nurse to travel to patient homes to administer home health and hospice services, supporting independence and comfort at end of life.

The ideal candidate will use sound clinical judgment, build rapport with patients, and collaborate with a multidisciplinary team to implement plans of care, with emphasis on education for patients and families.

Formación

• Active RN license in Kansas.
• Associate degree in Nursing is required; Bachelor’s preferred.
• Two years of home health or hospice experience preferred.
• BLS certification required; ACLS/BTLS preferred.

Responsabilidades

• Assess patient condition, document findings, and monitor vitals.
• Provide education to patients and families on home health procedures.

JOB DESCRIPTION:The Health at Home Registered Nurse is responsible for traveling to a patient’s home to administer home health and hospice services and helping patients maintain their independence or allow them to remain in their home at end of life. The right candidate will possess strong interpersonal skills that allow them to get to know their patients, build a rapport, and establish trust to provide high quality care while improving the patient’s quality of life and comfort. The home health/hospice nurse must be able to use sound clinical judgment to make decisions independently when required and accept direction and requests from patients. The home health/hospice nurse must be prepared to work in a variety of living environments and serve as an advocate for the patient.The Health at Home nurse provides a variety of tasks during patent visits depending on the specific plan of care and type of patient, duties include but are not limited to:Assess and document observations of patient conditionComplete evaluation, review medications, and obtain vital signsChanging of dressings as ordered and assess wound/healing progressProvide education to patients and families on proper home health strategies and proceduresCoordinate with the multi-disciplinary care team on progress related to the plan of careABOUT AMBERWELL HEALTH:Amberwell Health is a healthcare system dedicated to improving the health and wellness of the communities we serve, providing compassionate healthcare with clinical excellence.
