Schedule Details/Additional Information:

RN needed for in-home skilled nursing visits in Gurnee/Waukegan/Park City/Zion area. M-F 8:00am-5:00pm, weekend rotation every 3rd, on-call based on team rotation.

Position: REGISTERED NURSE HOME HEALTH

Location: FAR NORTHEAST SUBURBS

Hours: DAYS

This position is eligible for a $10,000 sign-on bonus, with 50% payable upon hire and 50% at 1 year, subject to taxes. Must be employed in this position at payout. Internal team members are not eligible. Must work one summer and one winter holiday. Rotation on an on-call schedule required.

Job and Unit Summary:

The Home Health division of Advocate Health Care is Illinois' largest home health and hospice provider. The RN will provide home care to the homebound, establishing strong patient and caregiver relationships to help patients reach their maximum potential. The role involves coordinating care, managing medical needs, and documenting care via point of care systems. Visits are made to private residences, which may not be accessible to those with disabilities.

The Advocate Home Health Nurse:

• Assumes responsibility for assessment, planning, implementation, and evaluation of nursing care.
• Sets holistic goals with patients and families.
• Leads at the unit level and demonstrates clinical mastery.
• Develops therapeutic relationships with patients.
• Ensures quality care and patient safety.

Pay Range: $37.50 - $56.25
