Job Responsibilities

• Provide and direct nursing care to patients in their homes, following physician prescriptions and legal regulations.
• Coordinate care plans with healthcare professionals and ensure continuity of care.
• Conduct initial and comprehensive nursing evaluations, develop care plans, and document accurately and timely.
• Perform patient assessments and develop appropriate care strategies.
• Verify patient eligibility and medical necessity for home health services.

License Requirements
