Job summary

Medi Home Health and Hospice, part of the Medical Services of America family, seeks a Full-Time Registered Nurse to provide skilled home health care in the Watauga County area, NC. You will assess patients, develop care plans with physicians, and support recovery in the comfort of patients’ homes.

The role emphasizes independence, patient-centered care, and collaboration with a supervisory director to improve quality of life and reduce unnecessary hospital visits.

Qualifications

• Active RN license in North Carolina.
• Completion of an accredited nursing program.
• At least 1 year of clinical nursing experience; home health preferred.

Responsibilities

• Provide one-on-one skilled nursing care to home health patients.
• Perform initial assessments and develop individualized care plans with physicians.
• Monitor and document patient progress and adjust plans as needed.
• Educate patients and families to support home-based care.
• Ensure care aligns with policies and physician orders.
• Participate in rotating on-call responsibilities.

We’re currently seeking a dedicated and experienced Full-Time Registered Nurse (RN) to join our growing team and provide skilled, patient-centered care to individuals in the Watauga County area.

As a Home Health RN, you’ll be a trusted guide in a patient’s recovery journey—offering support, clinical expertise, and compassionate care in the comfort of their home. Working under the direction of the Director of Professional Services, you'll help patients regain independence and improve their quality of life while avoiding unnecessary hospital visits.

What You’ll Do:

• Provide one-on-one skilled nursing care to home health patients.
• Perform comprehensive initial assessments and develop individualized care plans in coordination with physicians.
• Monitor, evaluate, and document patient progress and adjust care plans as needed.
• Deliver education and support to patients and families to empower them in managing care at home.
• Ensure all care provided aligns with agency policies, clinical guidelines, and physician orders.
• Participate in rotating on-call responsibilities to ensure continuity of care.

What You Bring:

• Completion of an accredited nursing program.
• Active Registered Nurse license in the state of North Carolina.
• At least 1 year of clinical nursing experience (Home Health experience strongly preferred).
• A strong sense of independence and a commitment to patient-centered care.
• Reliable transportation and valid driver’s license.
