Job summary

Medical Services of America, Inc. is seeking a dedicated Registered Nurse (RN) for Home Health in Laurens, SC.

This full-time role delivers skilled nursing care to patients in their homes, conducts initial assessments, develops care plans with physicians, and documents patient progress to support independence and avoid unnecessary hospital visits. You bring an accredited nursing program completion, SC RN license, and at least 1 year of clinical experience (home health strongly preferred).

Qualifications

• Completion of an accredited nursing program.
• Active South Carolina Registered Nurse license.
• Minimum 1 year of clinical nursing experience; Home Health preferred.

Responsibilities

• Provide one-on-one skilled nursing care to home health patients.
• Perform initial assessments and develop individualized care plans with physicians.
• Monitor, evaluate, and document patient progress and adjust plans as needed.
• Educate and support patients and families for home care management.
• Ensure care aligns with agency policies, guidelines, and physician orders.
• Participate in on-call rotation to ensure continuity of care.

Hourly Range: $36.00 – $42.00

We’re currently seeking a dedicated and experienced Full-Time Registered Nurse (RN) to join our growing team and provide skilled, patient-centered care to individuals in the Laurens, SC area.

As a Home Health RN, you’ll be a trusted guide in a patient’s recovery journey—offering support, clinical expertise, and compassionate care in the comfort of their home. Working under the direction of the Director of Professional Services, you'll help patients regain independence and improve their quality of life while avoiding unnecessary hospital visits.

What You’ll Do:

• Provide one-on-one skilled nursing care to home health patients.
• Perform comprehensive initial assessments and develop individualized care plans in coordination with physicians.
• Monitor, evaluate, and document patient progress and adjust care plans as needed.
• Deliver education and support to patients and families to empower them in managing care at home.
• Ensure all care provided aligns with agency policies, clinical guidelines, and physician orders.
• Participate in rotating on-call responsibilities to ensure continuity of care.

What You Bring:

• Completion of an accredited nursing program.
• Active Registered Nurse license in the state of South Carolina.
• At least 1 year of clinical nursing experience (Home Health experience strongly preferred).
• A strong sense of independence and a commitment to patient-centered care.
• Reliable transportation and valid driver’s license.
