Job summary

Avow Hospice in Naples, FL is seeking a registered nurse for home health care to assess patients, manage pain, and coordinate care. The RN uses the nursing process to tailor services to each family.

You will perform OASIS assessments, develop and adjust the plan of care, supervise aides, and educate families while following HIPAA guidelines.

Qualifications

• Graduate of an accredited nursing school.
• One year medical/surgical RN experience.

Responsibilities

• Initial and ongoing comprehensive assessment of the client’s needs, including OASIS assessments at appropriate time points.
• Initiating the plan of care and revising as necessary.
• Providing those services and/or treatments requiring substantial and specialized nursing skill.
• Counseling the client and family in meeting nursing and related needs.
• Initiating appropriate preventative and rehabilitative nursing procedures.
• Preparing clinical and progress notes.

• Informing the physician and other staff of changes in the client’s needs.

Job Summary

The RN utilizes the nursing process of assessment, planning, intervention, implementation andevaluation. The RN is skilled at assessment and management of pain and other complex symptoms,including education and support to patient and family. The mission of the home health team is to meetthe unique needs of clients and families with services tailored to assist them in achieving optimal level offunction.

Job Duties

• Initiating the plan of care and revising as necessary. The home health care plan isdesigned to the unique needs of clients and families with services tailored to assist themin achieving optimal level of function.

QualificationsEducation/Experience

Graduate of an accredited nursing school. One year medical/surgical RN experience.

Certificates, Licenses, Registrations

Valid Florida Driver’s License. Current Registered Nurse License to practice in the state of Florida Healthcare Provider CPR required upon hire and maintained.

Supervisory Responsibilities

May supervise LPNs and Home Health Aides as appropriate.

Computer Skills

Knowledge of basic typing skills, data entry and word processing software, database software andgeneral knowledge of e-mail.

Language Skills

Ability to read, analyze, and interpret common scientific and technical journals. Ability to respond tocommon inquiries or complaints from customers, regulatory agencies, or members of the businesscommunity.

Mathematical Skills

Ability to work with mathematical concepts such as probability and statistical inference. Ability toapply concepts such as fractions, percentages, ratios, and proportions to practical situations.

Reasoning Ability

Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpretan extensive variety of technical instructions in mathematical or diagram form and deal with severalabstract and concrete variables.
