Modern Machining Solutions Inc is seeking an experienced Home Health RN Case Manager for a field-based role serving Fort Worth, TX and the surrounding Tarrant County area. The ideal candidate is a clinically strong RN with recent home health case management experience, strong assessment skills, and the ability to manage an assigned patient caseload in the field.

Qualifications

• Active Texas RN license in good standing.
• Recent Home Health RN Case Management experience required.
• Strong understanding of Medicare home health, OASIS, and plan-of-care management.
• Ability to work independently with strong communication to clinical team.
• Valid driver’s license and ability to travel locally throughout Fort Worth / Tarrant County.

Responsabilités

• Manage assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate individualized plans of care with physicians, patients, families, and the interdisciplinary care team.
• Complete accurate, timely clinical documentation, including OASIS and visit notes.
• Monitor patient progress, identify changes in condition, and communicate updates to the team.
• Provide patient and caregiver education related to disease management, medications, safety, and plan-of-care goals.
• Coordinate care with therapy, social work, aides, physicians, and office/clinical leadership.
• Maintain compliance with home health regulatory, documentation, and quality standards.

Home Health RN Case Manager – Fort Worth / Tarrant County, TX

Territory: Tarrant County area - Hurst, Euless, Bedford, Grapevine, Irving

We are seeking an experienced Home Health RN Case Manager for a field-based role serving Fort Worth, TX and the surrounding Tarrant County area. This position is best suited for a clinically strong Registered Nurse with recent home health case management experience, strong assessment skills, solid documentation habits, and the ability to manage an assigned patient caseload independently in the field.

Key Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.

• Valid driver’s license, reliable transportation, and ability to travel locally throughout Fort Worth / Tarrant County.
• EMR experience preferred.
