As a Home Health Registered Nurse, you will:
- Provide admission, case management, and follow-up skilled nursing visits for home health patients.
- Administer ongoing care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager.
- Confer with physicians to develop and revise the initial plan of treatment based on orders and assessments, under supervision.
- Coordinate care with healthcare personnel and external providers.
- Report patient progress to physicians and Clinical Managers regularly.
- Implement care plans collaboratively with patients and families to address their needs.
- Supervise and document the work of Home Health Aides and related staff, ensuring quality and compliance.
- Discharge patients with proper documentation after consultation.
- Maintain comprehensive medical documentation, including updates, care plans, and communication with other disciplines.

Required Experience/Skills:
- Diploma, Associate, or Bachelor Degree in Nursing.
- At least one year of nursing experience preferred, with strong med-surg, ICU, ER, or acute care background.
- Home health experience is a plus.
- Valid RN license and CPR certification.
- Strong organizational and communication skills.
- Valid driver’s license, auto insurance, and reliable transportation.
Pay Range: $55.00 - $76.00 per visit/unit; $80,400 - $110,500 annually, depending on location and experience.
