Job summary

Azza HealthCare Agency is seeking a Home Care Registered Nurse to deliver person-centered nursing in clients' homes in Dunn, NC. You will assess, plan, and coordinate care, educate families, and ensure safe medication administration while collaborating with physicians and the care team.

You'll document in EMR, uphold infection control, and contribute to quality improvement initiatives in partnership with public-sector programs to improve patient outcomes.

Home Care Registered Nurse provides person-centered nursing care to clients in their homes. Key duties include performing comprehensive assessments, developing and updating individualized care plans, administering medications and treatments, and monitoring both chronic and acute conditions.

The RN coordinates with physicians and interdisciplinary teams—including behavioral health specialists, therapists, and social workers—to deliver evidence-based, outcomes-focused care. The role also includes educating patients and families on care needs and self-management.

Additional responsibilities include accurate documentation in an electronic medical record (EMR), promoting client safety and infection control practices, and supporting quality improvement efforts while collaborating with public-sector partners.
