Job summary

Personal Touch Home Health Care Services Inc. is seeking a Home Health Registered Nurse to provide field-based skilled nursing care to adult patients in their homes.

You will conduct comprehensive assessments, develop and manage individualized care plans, and deliver treatments with medication management. You will coordinate services with physicians, therapists, and aides, monitor patient conditions, support disease management and education, and document all activities in the EMR.

Qualifications

• Current nursing license (RN).
• Strong assessment, communication and coordination skills essential.
