Job summary

BAYADA Home Health Care is seeking a Full Time Registered Nurse (RN) for home health visits in Horry County, SC. The role supports MUSC Health at Home by BAYADA and involves following patient care plans and performing skilled nursing tasks in a home setting.

The RN will travel to designated geographic territories, document observations, and coordinate with the care team. SC RN licensure, one year of clinical experience, and ability to travel are required; home health experience preferred.

MUSC Health at Home by BAYADA has an immediate opening for a Full Time Registered Nurse, Home Health to provide home health visits for our MUSC Health at Home by BAYADA Office in Horry County, SC.

As a home health care RN, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.

Registered Nurse (RN) Responsibilities:

• Follow a designated care plan in accordance with patient's needs
• Make home visits to clients in designated geographic territories
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures
• Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
• Follow up with, execute, and properly document doctors' orders
• Perform client assessments as necessary
• Case management and coordination
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet

Qualifications for a Registered Nurse (RN):

• A current license as a Registered Nurse in SC.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care experience strongly preferred, but not required.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.
