Personal Touch Home Health Care Services Inc. is seeking a Registered Nurse to deliver 1:1 patient care in beneficiaries' homes, supporting an underserved community with a flexible schedule.

Build meaningful relationships while providing essential home health services within a collaborative team environment. Responsibilities include assessing needs, developing care plans, coordinating with families and interprofessional staff, and ensuring safety and quality in care delivery.

Qualifications

• RN license and Nursing Diploma/Associate's degree.
• Minimum of 2 years of experience; bachelor's degree preferred with 2-5 years of experience.
• Valid driver's license; willingness to cover all regions during on-call.
• Registration with the Family Care Safety Registry required.

Responsibilities

• Assessing patient needs, preferences, and barriers to care, considering cultural, emotional, and spiritual factors.
• Developing, implementing, and documenting individualized care plans in collaboration with patients, families, and the interprofessional team.
• Promoting respect, empathy, and cultural sensitivity during care delivery, ensuring patient-centered approaches.
• Monitoring patient condition changes, updating care plans, and communicating effectively with patients and team members.
• Ensuring evidence-based practice and quality outcomes while maintaining safety in all activities.
