We are seeking a dedicated and experienced Registered Nurse (RN) for a 13-week Home Health assignment in Bakersfield, CA. The RN will provide direct patient care in the home setting, following individualized care plans and collaborating closely with physicians, case managers, and caregivers.

Job Title: Registered Nurse RN - Home Health (13-Week Assignment)

Location: Bakersfield, CA

Assignment Type: Travel Contract

Duration: 13 Weeks

Start Date: 07/07/2025

Schedule: Day Shift - 8:00 AM to 5:00 PM

Shifts: 8 hours per day, 5 days per week

Weekends/Holidays: Not required

Pay Rate: 2500 to 2800 per week (based on experience)

Mileage: Reimbursed in addition to bill rate

Job Description:

We are seeking a dedicated and experienced Registered Nurse (RN) for a 13-week Home Health assignment in Bakersfield, CA. The RN will provide direct patient care in the home setting, following individualized care plans and collaborating closely with physicians, case managers, and caregivers.

Responsibilities:

• Deliver skilled nursing care to patients in their homes under physician's orders.
• Complete initial assessments and ongoing evaluations of patient needs.
• Develop, implement, and revise individualized care plans.
• Educate patients and families on disease process, medications, and care instructions.
• Administer medications, perform wound care, and manage IV therapy.
• Document patient assessments, interventions, and progress accurately and in a timely manner.
• Coordinate with interdisciplinary healthcare team to ensure quality patient care.
• Ensure compliance with all agency policies, procedures, and applicable regulatory requirements.
• Maintain communication with physicians and clinical team.
• Participate in case conferences and staff meetings as needed.
• Travel to patient homes within a designated territory
  
  

Qualifications:

• Active Registered Nurse (RN) license in the state of California.
• Current CPR or Basic Life Support (BLS OR HS-BLS OR RQI BLS) certification.
• Valid Driver's License and reliable personal vehicle.
• Current automobile insurance.
• Minimum of 1 year of recent home health experience preferred.
• Strong clinical and critical thinking skills.
• Excellent time management and communication skills.
• Ability to work independently in a fast-paced environment.
