Job Summary

The Registered Nurse in Home and Community Based Services provides and directs nursing and personal care to patients in their homes, as prescribed by the physician or authorized by the state/program, in compliance with applicable laws, regulations, and agency policies. The RN coordinates the care plan with other healthcare professionals involved and helps maintain continuity of care by planning and exchanging information with physicians, agency personnel, patients, families, case managers, and community resources.

Specific Job Duties/Responsibilities

• Provides high-quality clinical services within scope of practice and infection control standards, in accordance with the care plan and in coordination with the care team from admission through discharge.
• Completes clinical nursing assessments as required by federal and state program requirements and payers.
• Ensures patient eligibility and medical necessity for services as defined by payer source and agency policy.
• Develops and revises individualized care and service plans according to federal and state requirements with community providers.
• Ensures care plan frequency and duration meet patient needs and initiates revisions with physician approval.

Experience and License Requirements

• A minimum of one year RN experience preferred.
• Current RN licensure in the practicing state.
• Valid driver’s license, vehicle insurance, and access to reliable transportation.
• Current CPR certification.
