Job summary

St. Mary Home Care, an agency of Trinity Health At Home, seeks a full-time Home Health Registered Nurse Case Manager to provide in-home skilled nursing care in Bucks County. You will develop personalized care plans, administer medications, and support patient recovery in a home setting.

As part of Trinity Health At Home, you’ll join a growing team dedicated to compassionate, patient-centered care. Weekend rotations are required monthly with limited holidays, and a comprehensive orientation is

Qualifications

• Graduate of an approved nursing education program.
• Licensure as a Registered Nurse in the state of PA.
• Two years of professional home care nursing experience preferred.
• Must have current Driver’s license and reliable transportation.
• Ability to work a weekend rotation once monthly & 2 holidays annually.

Responsibilities

• Provide in-home skilled nursing care to patients by developing personalized care plans.
• Administer medications and support recovery and independence.
• Deliver one-on-one care in patients' homes.
• Collaborate with interdisciplinary team to coordinate patient care.

Minimum qualifications

• Graduate of an approved nursing education program
• Licensure as a Registered Nurse in the state of PA
• Two (2) years experience as a professional care nurse with home care experience preferred
• Must have current Driver’s license and reliable transportation
