We are seeking Registered Nurses (RNs) willing to travel regionally to conduct in-home Health Risk Assessments (HRAs) for Medicare Advantage members. The RN must be licensed in at least three of the following states: North Carolina, South Carolina, Texas, Illinois, Ohio, Michigan, Minnesota, Maryland, Georgia, Kansas, Wyoming,Idaho, New York, or Montana.

Position Responsibilities
As a Registered Nurse, you will:
???? Conduct comprehensive in-home Health Risk Assessments (HRAs) for Medicare Advantage members.
Review medical history, medications, lifestyle, and social determinants of health.
Perform physical and mental health evaluations and document findings in real-time.
???? Provide preventive care education without the need for ongoing patient management.
Travel regionally for in-home assessments.
Qualifications
️Active Registered Nurse (RN) license.
️ Minimum of 2 weeks per month, including five consecutive days for three months.
Proprietary EMR System – Simple, streamlined documentation via a company-provided iPad.
