What Will You Do?

• Serve as the Team Lead overseeing the day-to-day operations for a team made up of Nurses and Medical Assistants, while working with Physicians and Advance Practice Providers.
• Provide nursing care for the independent living residents on the Breton Woods Campus who choose to utilize the services of the Health Centre.
• Build & maintain trusting relationships with residents and their families, addressing resident and family concerns as needed.
• Coordinate scheduling of appointments to see the Physician.
• Assist in coordinating the transfer of residents to higher levels of care; completing required documentation in a timely manner.
• Collaborate with leadership in Independent Living along with managers in other areas of the organization such as assisted living, skilled nursing, Faith Hospice, Atrio Home Care, etc.

Education And Experience

• Requires completion of accredited RN program and licensure as an RN in the state of Michigan
• Minimum of 2 years of nursing experience; prior experience in emergency room or home care case management is highly valued.
• Proven experience exhibiting strong nursing assessment skills and the ability to make clinical decisions independently.
