Job summary

Nonotuck Resource Associates is seeking a dedicated Registered Nurse to provide oversight and support for Shared Living and Adult Family Care homes. The RN will work closely with individuals served, their families, caregivers, and service partners to develop personalized services and relationships.

This full-time role emphasizes hands-on coordination of intake, assessments, care plans, and on-site visits, with weekend coverage and driving to member homes as needed, in a collaborative team-based

Qualifications

  • Current MA Registered Nurse License.
  • Two years of experience with elderly or disabled adults.
  • Bachelor’s Degree preferred.
  • Valid driver’s license with clean RMV record.
  • Background check required.

Responsibilities

  • Intake assessments for new referrals and subsequent assessments.
  • Supervise, train, and evaluate caregivers to ensure high-quality care.
  • Develop and maintain individualized plans of care.
  • Conduct on-site visits and monitor health status.
  • Attend meetings with families, caregivers, and partners.
  • Report health status changes to physicians and document progress.

Registered Nurse (Full-time, Monday-Friday 40 hours/week)
Key Responsibilities
  • Intake and Assessments
    • Complete thorough intake assessments for new referrals and subsequent assessments as required.
    • Conduct nursing assessments and submit Prior Approval requirements as per members’ insurance coverage.
  • Supervision and Training
    • Provide supervision, training, and evaluation of caregivers to ensure high-quality care delivery.
    • Foster a collaborative and supportive environment within the caregiving team.
  • Individualized Plans of Care
    • Develop and maintain detailed Individualized Plans of Care tailored to each member’s unique needs.
    • Ensure plans are regularly reviewed and updated to reflect changes in health or circumstances.
  • On-Site Visits
    • Conduct on-site visits with each member per determined need and more often as required.
    • Monitor health status and general needs of all members and caregivers, making referrals as necessary.
  • Meeting Attendance
    • Attend various meetings fully prepared beforehand, as required.
    • Collaborate with team members, caregivers, families, and other pertinent parties.
  • Reporting and Documentation
    • Report changes in health status of any members to the member’s physician, director, and other relevant parties.
    • Complete nursing progress notes at each visit and fulfill other documentation requirements per service type.
  • Discharge Planning
    • Plan for and implement appropriate discharges or transitions of care in coordination with the healthcare team.
  • Education and Training
    • Provide ongoing educational information and training to all members and caregivers on health and aging.
    • Conduct member-specific trainings and address seasonal and current health/public health conditions.
  • On-Call System
    • Participate in the on-call system as scheduled, providing support as needed during non-business hours.
  • Weekend Availability
    • Work weekends as required several times throughout the year to meet the needs of the individuals supported.
  • Driving Requirement
    • Ability to drive a motor vehicle to the homes of people served.
  • Teamwork and Essential Duties
    • Collaborate effectively within the agency, office, team, and with all relevant stakeholders.
    • Perform any and all duties deemed essential to assist the agency in achieving its mission.
Qualifications
  • Current MA Registered Nurse License.
  • Two years of recent experience working with elderly or disabled adults, or individuals diagnosed with ID/D, ABI, or chronic conditions affecting their ability to live safely in the community.
  • Bachelor’s Degree preferred.
  • Background Check Required.
  • Valid driver’s license with an excellent RMV record (subject to RMV background check).
  • Computer and organizational planning skills.
  • Strong interpersonal and communication skills.
  • Ability to work collaboratively in a team environment.
  • Prior community experience with the Department of Developmental Services, Adult Foster Care Services, and HCBS waivers ideal.
