All care is provided based on the concepts inherent in the model of care for TCH, which promotes an ongoing partnership between patients, families, and the healthcare team. The care is culturally based and age-specific. The RN adheres to the American Nursing Association (ANA) code of ethics and the scope of practice described in the Ohio Nurse Practice Act.

  3. Use appropriate handoff methods to communicate pertinent data regarding patient's physiological and psychological status with team members.

  3. Establish care priorities, discuss plans during shift reports, and participate in care conferences.
  4. Initiate discharge planning upon admission and make appropriate referrals based on anticipated length of stay.

Implementation - Provide evidence-based, quality care based on assessed needs and standards, adhering to hospital policies to achieve set goals.

  1. Perform and delegate nursing actions based on orders, demonstrating critical thinking and accountability.
  2. Provide ongoing, individualized education to patients and families, collaborating with the multidisciplinary team.
  3. Review physician orders critically, make appropriate referrals, and contact other health professionals as needed.
  4. Respond effectively to emergency situations requiring immediate action.
  5. Implement infection control measures and use protective devices to maintain safety.

Evaluation - Continuously evaluate nursing practice against standards and patient outcomes, adjusting care plans as necessary.
