Qualifications

Responsibilities

• Supervise and support CNAs under the Director of Nursing.
• Collaborate with the IDT to assess, plan, implement, and evaluate care plans.
• Ensure baseline and periodic assessments are completed on time.
• Prepare for physician rounds and document concerns for review.
• Initiate consultations or referrals as needed.
• Maintain medical documentation, evaluations, and reports.
• Deliver nursing care within policy and standards.
• Monitor residents for changes in condition and notify providers.
• Perform other duties as assigned by the Director of Nursing.
