Responsibilities

• Performs daily rounds on all patients, coordinating care and ensuring quality service.
• Develops a care plan based on assessment of physical, psychological, social, and rehabilitative needs.
• Assesses patient condition, sets goals, prescribes appropriate nursing actions, including discharge planning.
• Intervenes for pain and monitors changes in patient condition with escalation to clinical team as needed.
• Records observations accurately and ensures timely referrals and coordination with the care team.
