Job summary

West State Street is seeking an experienced Registered Nurse (RN) to deliver high‑quality care in a long‑term care setting. Under supervision, you will conduct assessments, create care plans, and coordinate with a multidisciplinary team to achieve optimal patient outcomes.

You will supervise LPNs and CNAs, document care in real time, and ensure compliance with HIPAA and safety protocols. A BSN/ADN with active WV licensure and BLS/CPR is required, with opportunities for career growth.

Qualifications

• Graduate of an accredited School of Nursing (ADN or BSN).
• Active RN license in the state.
• BLS/CPR certification required.
• Experience in long‑term/geriatric care preferred, strong clinical skills.

Responsibilities

• Provide holistic patient care through assessments, diagnoses, care plans and interventions.
• Lead and delegate to LPNs and CNAs ensuring patient safety and quality care.
• Collaborate with physicians and care teams for effective discharge planning.
• Maintain accurate, real‑time clinical documentation using PCC.
