PowerBack Rehabilitation in West Virginia is seeking a dedicated Registered Nurse (RN) to deliver high-quality nursing care for patients in a long-term care setting. Under supervision, you will assess patients, plan and implement individualized care, and collaborate with the interdisciplinary team to achieve optimal outcomes.

You will supervise LPNs and CNAs, maintain PCC documentation, and ensure regulatory compliance while supporting families and ensuring safe transitions of care.

Qualifications

• Education: ADN or BSN from an accredited nursing program.
• Licensure: Active RN license in good standing.
• Certifications: BLS/CPR certification.
• Clinical Experience: Long-term care, post-acute, rehabilitative, or geriatric nursing preferred.

Responsibilities

• Perform comprehensive clinical assessments and develop care plans.
• Administer medications and treatments; monitor patient responses.
• Coordinate with physicians and interdisciplinary teams for safe discharge planning.
• Lead and mentor LPNs and CNAs; delegate tasks per state acts.
• Maintain accurate real-time documentation in PCC EHR.
• Ensure HIPAA compliance and safety protocols.
