Job summary

Affinity Home Health Care Inc is seeking a Registered Nurse for a role in a long-term and post-acute care facility in Indiana. You will assess, plan, implement, and evaluate individualized resident care plans, and administer medications and treatments with accuracy.

You will collaborate with an interdisciplinary team, educate residents and families, mentor CNAs, document diligently, and uphold safety standards while promoting dignity, recovery, and compassionate, resident-centered care.
