Job summary

Proforma Health is seeking a Registered Nurse to join our skilled nursing and rehab team. You will perform comprehensive resident assessments, administer medications, and support rehabilitation programs with a multidisciplinary team.

You will maintain accurate EMR documentation, monitor residents' conditions, educate residents and families on health matters, and promote dignity, safety, and professional growth in a compassionate, resident-centered environment.
