Job summary

Prestige Home Care is seeking a passionate registered nurse to assume responsibility for individual medical case management of agency clients on a case-by-case basis.

You will ensure the quality of medical care meets DDD and agency standards while collaborating with CBS agency administrative staff and hospital/medical professionals as needed. Duties include coordinating services, maintaining client confidentiality, and advocating for clients' best interests.

Qualifications

• RN license in New Jersey required.
• Experience with case management and coordinating medical services.

Responsibilities

• Monitors health of individuals through review of professional medical reports.
• Assesses educational needs of staff and arranges in-service training.
• Advocates for clients and acts as a medical liaison with hospital staff.
• Maintains confidentiality and security of client information per policies.
• Collaborates with team members and community agencies to plan and coordinate services.
• Performs other duties as assigned by leadership.

Looking for a passionate and compassionate RN to a ssumes responsibility for individual medical case management of agency clients, on a case-by-case basis. Responsible for ensuring the quality of medical care provided meets DDD and Agency standards while working cooperatively with the CBS agency administrative staff

Responsibilities;

• Monitors the health of the individuals through review of Professional Medical Reports.
• Assesses the educational needs of the staff and conducts or arranges for appropriate in-service training programs.
• Advocates in the best interest of the clients at all times, acting as a medical liaison between staff and hospital/medical professionals when necessary.
• Maintains the confidentiality, security and integrity for the management of client information according to agency and DDD policies
• Communicates and collaborates with other team members and community agencies in planning, implementing and coordination and evaluating all services necessary to insure comprehensive care.
• Performs other duties as may be assigned by Director of Residential Services and/or the Executive Director.
