Qualifications

• ADN or BSN required.
• Colorado RN license in good standing.
• 2-4 years related experience or equivalent

Responsibilities

• Provides general medical care and treatment to patients under the direction of medical providers.
• Float between clinics, primarily Monfort Family Clinic in Evans, CO.
• Performs disease case management for patients with chronic conditions and LTBI.
• Performs medication reconciliation and preventive care activities.
• Maintains complete and accurate patient records and coordinates with healthcare team.
