Wentworth-Douglass Hospital is seeking an experienced RN to join our primary care float team, conducting Medicare Annual Wellness Visits across 11 offices including Dover, Barrington, Lee, Portsmouth, Rochester, and South Berwick. You will travel to a different office each day, performing preventive screenings and coordinating care in a dynamic ambulatory setting.

This role offers 10-hour shifts, four days per week (40 hours) Tuesday through Friday, with early start times.

Qualifications

  • Associate's Degree in Nursing required.
  • Bachelor's Degree in Nursing preferred.
  • Registered Nurse (RN) license required.
  • Basic Life Support (BLS) preferred.
  • 3–5 years of clinical RN experience.
  • Ambulatory/outpatient care experience preferred.

Responsibilities

  • Deliver direct patient care and support, ensuring high nursing standards.
  • Conduct patient assessments, devise care plans, and assist in outpatient procedures.
  • Educate patients and families on treatment plans, medications, and post-procedure care.
  • Collaborate with healthcare professionals to coordinate comprehensive patient care.
  • Administer medications, perform IV therapy, and maintain accurate patient records.
  • Uphold infection control protocols and maintain a safe environment.

This individual will travel to a different office each day, independently conducting Medicare Annual Wellness Visits and completing preventative screenings. The schedule rotates on a biweekly basis, with the RN receiving advance notice of their assigned office locations. Practice locations include:- Dover, NH- Barrington, NH- Lee, NH- Portsmouth, NH- Rochester, NH- South Berwick, MESchedule: 10-hour shifts, 4 days per week (40 hours), Tuesday through Friday, with a 7:00 AM–5:00 PM schedule on Tuesdays.The ideal candidate will have at least 3–5 years of RN experience, with previous primary care nursing experience strongly preferred. They should be comfortable working independently, adapting to different clinical environments, and traveling between multiple office locations on a rotating basis. Experience with Medicare patients and preventative care would be beneficial. Candidates with VNA, hospice, or end-of-life care experience may be particularly well suited for this position, as they are often familiar with the Medicare population and the unique care needs of these patients.Summary:Responsible for patient care and support within the outpatient setting through providing compassionate care, assisting in procedures, and collaborating effectively in a dynamic ambulatory care environment.Does this position require Patient Care?
