Job summary

Elevate Care is seeking compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to join our dedicated healthcare team. You will provide high-quality care, collaborate with a multidisciplinary team, and support residents' comfort and well-being.

Qualifications

• Current RN or LPN license active and in good standing.
• CPR and ACLS certification.
• Strong clinical assessment and critical thinking skills.

Responsibilities

• Provide comprehensive patient care and implement care plans for residents.
• Administer medications and treatments as prescribed with safety.
• Collaborate with physicians, therapists, and other professionals to coordinate care.
• Monitor patients’ vital signs and overall health status.
• Educate patients and families about care plans and health management.
• Maintain accurate documentation of care and changes in condition.
