Elevate Care Windsor Park in Chicago, IL is hiring Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to deliver compassionate, resident-centered care within a multidisciplinary team. You will assess, plan, and implement care, administer medications, monitor vital signs, educate residents and families, and maintain accurate records.

Your Responsibilities:

• Comprehensive Patient Care – Assess, plan, implement, and evaluate care plans for residents based on their health needs.
• Administer Medications & Treatments – Provide medications and treatments as prescribed, ensuring accuracy and patient safety.
• Collaborate with Healthcare Team – Work closely with physicians, therapists, and other professionals to coordinate care.

Qualifications:
